PROBATE NOTICE TO CREDITORS (RCW 11.40.030). The Personal Representative named below has been appointed as Personal Representative of this estate. Any person having a claim against the Decedent must, before the time the claim would be barred by any otherwise applicable statute of limitations, present the claim in the manner as provided in RCW 11.40.070 by serving on or mailing to the Personal Representative’s attorney at the address stated below a copy of the claim and filing the original of the claim with the Court. The claim must be presented within the later of: (1) Thirty days after the Personal Representative served or mailed the notice to the creditor as provided under RCW 11.40.020(3); or (4) four months after the date of first publication of the notice. If the claim is not presented within this time frame, the claim is forever barred, except as otherwise provided in RCW 11.40.051 and 11.40.060. Notice of Community Forums Regarding Levies
Notice is hereby given that Three Rivers Hospital will host seven community forums for the purpose of providing hospital district residents with information about two levy propositions appearing on the August 2, 2016 election ballot.
The forums will take place as follows:
• 5:30-6:30 p.m. Tuesday, May 17, 2016 at Liberty Bell High School, 24 Twin Lakes Road, Winthrop WA 98862.
• 7-8 p.m. Tuesday, May 17, 2016 at TwispWorks Education Station, 502 S. Glover St., Twisp WA 98856.
• 6-7 p.m. Tuesday, May 24, 2016 at Three Rivers Hospital’s Hillcrest Administration Building, 415 Hospital Way, Brewster WA 98812.
• 6-7 p.m. Tuesday, June 21, 2016 at Mansfield High School, 491 Road 14 NE, Mansfield WA 98830.
• 5:30-6:30 p.m. Thursday, June 23, 2016 at Bridgeport High School, 1220 Kryger Ave., Bridgeport WA 98813.
• 5:30-6:30 p.m. Monday, July 11 at the Douglas Okanogan County Fire District 15 Station in Pateros, 191 Industrial Way, Pateros WA 98846.
• 6-7 p.m. Wednesday, July 27, 2016 at Three Rivers Hospital’s Hillcrest Administration Building, 415 Hospital Way, Brewster WA 98812.
A quorum of the Three Rivers Hospital Board of Commissioners may be in attendance at any of the forums, but no hospital business will be conducted.

NOTICE OF APPLICATION SHORELINE SUBSTANTIAL DEVELOPMENT PERMIT
WIN SSDP 16-2 NOTICE IS HEREBY GIVEN THAT James Pigott, 1405 42 Ave, Seattle, WA 98862 has submitted application for a Shoreline Substantial Development Permit to demolish an existing structure and construct a park at 253 Riverside Avenue. The project is within the shoreline area of the Chewuch River, within the urban shoreline designation. No structures will be placed, nor will work occur below the ordinary high water mark, nor within the conservancy area. The project is located on tax parcels 2780012001 and 2780012101, in the Town of Winthrop.
Information Available: A JARPA form, SEPA Checklist and other application materials are on file at Winthrop Town Hall, 206 Riverside Ave. Persons wishing to view project information or receive notice of the action taken on the application may contact Winthrop Town Hall between the hours of 9 a.m. and 4 p.m., Monday – Friday. SEPA Determination: A threshold Determination of Nonsignificance was issued for the project on May 9, 2016. The comment period for the SEPA Checklist and Threshold Determination ends on June 3, 2016. The Town of Winthrop (lead agency) has determined that this project does not have a probable significant adverse impact on the environment. An environmental impact statement is not required under RCW 43.21C.030(2)(c). This decision was made after review of the information described above, which is available upon request. The DNS has been issued under WAC 197-11-340(2).
Comments: All persons are welcome to submit written comments concerning this application and the related SEPA Checklist and Threshold Determination. Comments or requests for notification of the action taken on the application must be submitted to the Town Planner, Town of Winthrop, P.O. Box 459, Winthrop, WA 98862, no later than 4:00 pm, June 24, 2016.
This notice is given pursuant to RCW 90.58.140, Winthrop Shoreline Master Program Chapter 7.09 & Winthrop Municipal Code Chapter 19.06.
